FIDAP Registration
Use this form to request access to the FIDAP platform. Access is limited to Inclusiv members that are up-to-date on the payment of their dues for the current policy year. If you are interested in becoming an Inclusiv member, please contact membership@inclusiv.org or fill out our online application.
After completing this form, it can take up to two business days to process your request. Please do not fill out this form if you will not be the administrative user on the FIDAP platform.
